「出席活動證明書」申請表

1.    Volunteer can apply for a Certificate of Attendance from V Foundation upon completion of 2 service hours or above for one V Foundation event. (excluding V are here mentorship programme)
義工於任何V慈善基金活動完成2個服務小時或以上方可申請「出席活動證明書」(V愛共行師友計劃除外)。

2.    It takes at least 10 working days for V Foundation to process the application, and the certificate will only be issued after the event.
V慈善基金需要至少10個工作天處理申請，證明書於活動完結後方會發出。

3.    Volunteer services team may contact the applicant for the purpose of information verification.
義工事務小組將按需要與申請者聯絡以核對資料。

4.    The purpose of collecting personal particulars in this form is for processing the application, notification of application progress and record keeping purpose.
